WHAT

Gain the skills & core competencies to work as a Community Health Worker (CHW) or in other roles under the umbrella of Community Health, e.g.

  Patient Navigator, Health Assister, Recovery Coach, Outreach Worker, Home Visitor, Patient Advisor, Patient Advocate. 

 

This 9-week course is offered through combined virtual interactive classroom and independent off-line study, with self-paced homework.

WHEN

Class Meeting Dates & Times

Wednesdays @ 9 AM - 3:30 PM on the following dates: 

  • February: 2, 9, 16 & 23
  • March: 2, 9, 16, 23 & 30

 

REQUIREMENTS

  • Attend: 9 total virtual class sessions (includes two-day MI Training) 
  • Complete assigned readings and assignments between scheduled classes
  • Must have computer/internet access, ability to use Zoom, and be 18 or older 

 

FUNDING SUPPORT

  • Region 7 Integrated Delivery Network (IDN) Partners- This course, including manuals and materials, are funded through IDN Workforce Development funds and are provided at no-cost to Region-7 IDN Partner Organization staff members.

The preparation of this training was financed under a Contract with the State of New Hampshire, Department of Health and Human Services, with funds provided in part by the State of New Hampshire and/or such other funding sources as were available or required, e.g., the United States Department of Health and Human Services.

 

  • Limited full scholarships are also available by request to individuals seeking to become a CHW, or individuals who are currently doing Community Health work and have not taken a CHW training previously. There are required questions that must be answered to complete a scholarship request and incomplete requests will not be considered.
